Formstack

Formstack (now branded Intellistack) is a no-code workplace productivity suite built around collecting data and automating document workflows. Its flagship Formstack Forms lets anyone build powerful online forms and surveys — with conditional logic, calculations, payments, and integrations — to collect data without coding. Alongside forms, Formstack Documents auto-generates documents (contracts, proposals, PDFs) from that data, and Formstack Sign adds e-signatures, so organizations can run end-to-end processes from data capture to signed document without manual work.

The platform's value is workflow automation for data-heavy processes. Forms capture information with logic and validation; that data flows into generated documents via merge templates; and Sign collects legally binding signatures — automating things like onboarding, approvals, agreements, and requests. Formstack integrates with Salesforce, Google, and hundreds of other tools (and offers Formstack for Salesforce), supports HIPAA compliance for healthcare, and provides analytics and routing. By combining forms, documents, and signatures, it replaces stitched-together point tools for common back-office workflows.

Formstack serves marketing, operations, HR, healthcare, and IT teams that need no-code data collection and document automation. It does not offer a free plan (a free trial is available); the entry Forms tier is around $83/month billed annually ($99 monthly) for one user and limited forms, with Suite/Teams tiers around $225 per user/month and custom Enterprise pricing (tiers were renamed Starter/Teams/Pro after the 2025 rebrand). It competes with Jotform, Typeform, SurveyMonkey, and DocuSign, differentiating on combining forms, document generation, and e-signatures in one suite.
